Why cant we conceive?

My boyfriend and I have been trying to conceive for 2 years now and nothing has happened yet. He is stubborn and refuses to go see a dr for help. Im pretty sure he has low sperm count because it runs in his family and he smokes a pack a day. He also used heroin awhile ago. What can we do to conceive if he doesn't want to go see a dr to get help? Are there at home sperm test kits we can buy to determine it ourselves and what are the best things for him to do to raise it. Quitting smoking is out of the question but he will cut back.
